- Pay Rate: $15-$18/hour, based on experience and education
As a Health and Wellness Navigator, you'll connect clients with resources to improve their well-being and stabilize their situations. This role is perfect for someone with social service experience who thrives on making a difference and is also highly skilled in clerical and data entry tasks.
Key Responsibilities:
- Maintain detailed and accurate client records in databases
- Manage client data for reporting and compliance purposes with precision
- Develop service plans and ensure follow-through on client appointments
- Provide compassionate support to clients and address social determinants of health
- Communicate effectively to build strong relationships with diverse communities
- Represent the program professionally in the community
- 18 years old with a valid driver's license and proof of insurance
- Strong data entry and clerical skills (accuracy and attention to detail are essential!)
- Proficiency in using computers and learning new software
- Prior experience in social services or working with the community is a plus
- Commitment to completing health navigation training (provided)
- Positive, organized, and passionate about helping others
- Familiarity with Stark County resources is preferred
